MEDICAL HISTORY

To be answered upon arrival to the hospital

Medical:

I, the Owner/Authorised Representative of the above mentioned Pet, have the full authority and right to execute this form and to provide the necessary authorization and consent required herein. I hereby authorize Zabeel Veterinary Hospital to perform diagnostic, therapeutic and/or surgical procedures described above or any procedure deems necessary by Zabeel Veterinary Hospital on the Pet. The nature of such services has been verbally described and explained to me and my questions have been answered to my full satisfaction. I acknowledge that no guarantee can ethically or professionally be made or given to me regarding the possible outcome, effect or cure of the treatments, services and/or procedures perform on my Pet. If anesthesia or sedation is required, I understand and accept that such procedures are always inherent with different levels of risks. I authorise Zabeel Veterinary Hospital to use sedatives and pain control medications on the Pet as and when deem necessary by Zabeel Veterinary Hospital. I also authorise Zabeel Veterinary Hospital’s staff in an emergency situation, to follow through with procedures that are necessary for the wellbeing of my Pet on a continuing basis until further communication with me is possible and until I instruct otherwise in writing. I understand that there could be possible complications associated with any anesthetic or surgical procedure. I have been informed of all possible complications, side effects and even the risk of death every time an anesthetic or surgical procedure is used or performed and I acknowledge and accept such possibility. I also acknowledge these risks, complications and/or side effects and understand that Zabeel Veterinary Hospital and its staff will try to minimize such risks, complication and/or side effects to the extent reasonably possible. I will not hold Zabeel Veterinary Hospital and its staff liable for any incidents, complications, injury, death and/or any disease or infection that may arise pursuant to the treatments, services and/or procedures perform on my Pet.

Hospitalization:

I understand that if my Pet is not vaccinated or if his/ her due date for vaccination has expired, my Pet may be at risk of contracting infectious and life-threatening diseases. If such case occurs then my Pet will be kept in a quarantine section to prevent potential risk of cross contamination or transmission of infectious diseases to other animals. I thereby take full responsibility for infectious disease my Pet may contract during its stay at Zabeel Veterinary Hospital.

Vaccination:

Most pets will not have any side effects from vaccines. The most common side effects are local pain/swelling at the site of the injection, mild lethargy and low-grade fever. These usually resolve on their own within 24 to 48 hours and do not require treatment. More severe reactions are less common, but can present such as vomiting, diarrhea, facial swelling, and hives. If my Pet shows signs of such reaction, I shall seek immediate medical attention. With any vaccine, anaphylaxis (a very severe and potentially fatal hypersensitivity reaction) is a risk but is very rare. Also, while there is no direct cause and effect relationship between vaccines and certain auto- immune diseases, this continues to be investigated and research performed within the industry. I hereby, understand and acknowledge the above and shall not hold Zabeel Veterinary Hospital and/or its staff liable for any such reactions and/or side effects and I understand that such reaction and/or side effect cannot be accurately predicted.

Financials

I understand that there may be unforeseen complications and that further treatment or changes to the treatment plan that may be necessary during the hospitalization of my Pet. I assume full financial responsibility for all the treatments, procedures and/or services rendered, and agree to pay all the fees at the time of my Pet’s discharge or demise and upon the production of the invoice. If I neglect to pick up my Pet, Zabeel Veterinary Hospital shall have the right to impose additional charges for the additional care to be provided to the Pet and if the Pet is still not pick up after five (5) days of written notice that it is available for release, it may be considered abandonment of my Pet, and in such event, I hereby, authorise Zabeel Veterinary Hospital, at its absolute discretion and without any liability, to sell or dispose of my Pet in any manner as it deems appropriate without the need to provide any further notice to me or to obtain any court order. Abandonment does not release me from my obligations for payment of all treatments, procedures and/or services rendered. Zabeel Veterinary Hospital reserves its right to take any appropriate steps to recover any unpaid fees.

This form and the relationship between the parties are governed by the laws of the Emirate of Dubai and the applicable federal laws of the UAE. Dubai Courts shall have exclusive jurisdiction.
